Accident Investigation and Root Cause Analysis (RCA)
Course Description
Participants learn the basic steps of conducting an effective investigation. We also introduce the Five Why methodology to determine the root causes of an incident. In addition, they will practice applying the Five Why Methodology to an accident scenario to identify all levels of causation as well as determining actions to prevent recurrence.
