Root Cause Analysis (RCA)
Course Description
Participants will review the levels of accident causation, the “Five Whys” analysis method, and categories of root causes. In addition, they will practice applying the “Five Whys” to accident scenarios to identify all levels of causation and determining actions to prevent recurrence.
