Affirmative Action – Disability self-ID form renewed

Federal contractors and subcontractors are required to use the current form to ask applicants and employees whether they have a disability. These employers must use the data they collect to assess their progress in recruiting and employing individuals with disabilities. The only change to the form is that the expiration date in the upper right corner is now 1/31/2020. You can download the form, which is available in multiple languages, from the OFCCP’s web page on voluntary self-identification of a disability. Covered federal contractors should begin using this updated version now. For more information, and to see examples of additional self-identification inquiries that are required regarding race/ethnicity, gender, and protected veteran status, see our Model Form, “Invitation to Self-Identify”.

On the very last day before it would have expired, the Office of Federal Contract Compliance Programs (OFCCP) posted an updated copy of its mandatory form for gathering disability data from workers.
